The Respiratory@Home service supports and monitors patients with respiratory conditions whilst they remain in their own homes. The virtual ward provides an alternative pathway to hospital admission and a safe discharge pathway for patients who require ongoing monitoring.
We take pride that our patients and their experience are at the heart of this service, making the process as easy and straightforward as possible. Upon receipt of referral, patients are contacted by our team to be onboarded into the service. Soon after, they receive a pulse oximeter and an information pack in the post that shows them how to collect their oxygen recordings so our team of care coordinators and GPs can monitor and support them.
Referral Criteria:
Adults (18 years and over) with confirmed or suspected acute respiratory illness, including COVID, who are stable or improving but require ongoing monitoring that can be safely provided in their home or usual place of residence.
